{"created":"2023-05-15T13:10:33.925005+00:00","id":492,"links":{},"metadata":{"_buckets":{"deposit":"398880a7-fb89-449e-b8af-1bd4060a4093"},"_deposit":{"created_by":12,"id":"492","owners":[12],"pid":{"revision_id":0,"type":"depid","value":"492"},"status":"published"},"_oai":{"id":"oai:rcakita.repo.nii.ac.jp:00000492","sets":["50:20:52"]},"author_link":["1287","1173","843","1294","1282","1295"],"item_10002_biblio_info_7":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2022-03-31","bibliographicIssueDateType":"Issued"},"bibliographicPageEnd":"11","bibliographicPageStart":"1","bibliographicVolumeNumber":"26","bibliographic_titles":[{"bibliographic_title":"日本赤十字秋田看護大学・日本赤十字秋田短期大学紀要"},{"bibliographic_title":"Journal of Japanese Red Cross Akita College of Nursing and Japanese Red Cross Junior College of Akita","bibliographic_titleLang":"en"}]}]},"item_10002_description_19":{"attribute_name":"フォーマット","attribute_value_mlt":[{"subitem_description":"application / pdf","subitem_description_type":"Other"}]},"item_10002_description_24":{"attribute_name":"記事種別","attribute_value_mlt":[{"subitem_description":"実践報告","subitem_description_type":"Other"}]},"item_10002_description_5":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"要旨\n目的:臨床判断の基礎的能力を育成するために「看護過程」で用いた紙上事例に基づくシミュレーション教育を実施し、得られた学生の経験知から教育的示唆を得ること。\n方法:看護過程で用いた循環器疾患の紙上事例に基づいて実施した離床支援の場面におけるシミュレーション教育における学生の学びを、臨床判断の枠組みに沿って整理した。\n倫理的配慮:対象者へ口頭で研究目的・方法を説明して承諾を得た。\n結果:急性期看護実習の履修者7名が参加した。離床支援シミュレーションで得られた学びは、「気づき」として【離床支援に伴う患者のめまい・気分不快】を含む2項目、「解釈」として【体位変換による循環動態の変化】を含む2項目、「反応」として【離床前後のバイタルサインの比較】を含む6項目、「省察」として【急性大動脈解離の再燃と気分不快の判別】【離床中の観察と会話の並行】を含む8項目、経験知として【急性大動脈解離の再燃に対する不安緩和】、【コミュニケーションによる苦痛の理解】を含む6項目が示された。\n考察:「看護過程」による病態生理や回復過程に関する学びは、学生が離床支援に伴う患者の変化の重症度・緊急度の査定を助けるものであったと考えられる。併せて、コミュニケーションについて教授することが重要である。\n結論:臨床判断の基礎的能力を育成するためのシミュレーション教育の学びは、患者の緊急度・重症度を査定するための観察、回復過程に応じた離床支援、コミュニケーションによる不安・苦痛の緩和に集約された。\n\nAbstract:\nPurpose: Conduct simulation education using paper-based patient case studies in the “nursing process” to develop basic skills for clinical judgment, and to provide educational suggestions relative to student learning.\nMethod: Simulation-based education was conducted using case studies of cardiovascular diseases patients to develop student learning of clinical judgement skills in line with the clinical judgment.\nEthical framework consideration: The purpose and method of the research was verbally explained to the subjects with the aim of obtaining their consent.\nResult: The results showed that seven students who took the acute nursing training learned the following from the bed leaving support simulation: The results showed that seven students who took the acute nursing training learned the following from the bed leaving support simulation: (1) “Noticing”: two items including “patient’s dizziness and mood discomfort associated with bed leaving support.” (2) “Interpreting”: two items including “change in hemodynamics associated with initial wake-up.” (3) “Responding”: six items including “comparison of vital signs before and after getting out of bed.” (4) “Reflecting”: eight items including “discrimination between relapse of acute aortic dissection and mood discomfort,” and “parallel observation and conversation while getting out of bed.” (5) “Experience knowledge”: six items including “responding to anxiety about a relapse of the disease” and “Understanding of pain due to communication.”\nDiscussion: It is probable that learning about pathophysiology and the recovery process through the \"nursing process\" helped students determine the severity and urgency of changes in patients associated with getting out of bed. At the same time, it is important to teach communication.\nConclusion: In order to develop the basic ability of clinical judgment, the learning of simulation education concentrated on observation skills for assessing the urgency and severity of patients, support for getting out of bed according to the recovery process, and relief of anxiety and pain through
